If you are a new patient, we ask that you bring your insurance card and a photo ID to your appointment. In addition please bring the completed new patient forms. If you are an existing patient, please inform us immediately if there are any changes with your insurance. We will also request an update on insurance information approximately every six months. We file primary insurance forms from our office. We do not file secondary insurance, except for medicare recipients. Our practice does not accept worker’s compensation, nor do we file third party claims (i.e. Auto Accidents). At the time of service, you will be responsible for all fees that are not covered by your insurance, including co-pays, co-insurance, deductibles and non-covered services received. The co-pay cannot be waived by our practice, as it is a requirement placed on you by your insurance carrier. Campbell Family Practice accepts the following major insurance policies below.
